Colleen Miller & Melinda Huffman, Principals of Miller & Huffman Outcome Architects, LLC, Winchester, Tennessee, bring 60 years combined healthcare administrative, clinical, and regulatory experience and expertise to the industry.  In 2005, Miller & Huffman began servicing the home health, DME and product vendor & manufacturing industries as they prepared for Pay for Performance (P4P) and an outcome-based reimbursement environment.  Recognizing the void in sound Care Transition and Disease management products and systems for the most costly chronic conditions, Miller & Huffman embarked on the development of evidence-based and outcome-driven clinical tools, publications, applications, and software systems to guide and support providers and vendors alike.

 

Their first outcome-based offering: Home Health Evidence-based Fall Prevention, distributed by the Briggs Corporations in 2006.  In 2007, they co-founded the National Society of Health Coaches (NSHC) to provide and promote evidence-based health coach education, training and certification of licensed practitioners, launching the first program of if its kind in the United States developed by clinicians for clinicians. 

 

In 2008, Miller & Huffman launched their premier wound care Telehealth subscription service WOCN-Connect and its revolutionary wound outcomes software, along with their Disease Management BluePrints.   In 2011, Miller & Huffman published their 2nd Edition Health Coach Certification Program: Evidence-based Health Coaching for Healthcare Providers, containing the nation’s first Clinical Model of Evidence-based Health Coaching.

 

Miller & Huffman Consulting provides unparalleled Clinical Due Diligence, Regulatory Guidance and Operations Management, Quality Management and Legal Case Review expertise and services to providers and to Venture Capital and Private Equity Firms engaged in healthcare mergers and acquisitions.

Colleen Miller, RN, BS, CSBC, CHC, Principal of Miller & Huffman Outcome Architects, LLC is an author, writer, speaker, Certified Small Business Consultant and Certified Health Coach specializing in: Homecare & Hospice Regulations and Operations, Clinical Due Diligence Audits (M&A, Private Equity/Venture Capital) Clinical Compliance, Strategic Planning and Positioning, Clinical Outcome System Designs & Integration including Telehealth.

 
She has 30+ years health industry experience and has authored many publications and articles including: ICD-9-CM Coding Quick Reference for Home Health Clinicians and Managers 1st and 2nd Editions, co-authored Evidence-based Interventions & Decision Support for the Management of Chronic Conditions in OASIS C, and BluePrints for Chronic Condition Management. She is also the Researcher/Developer of the Teletriage Symptom Reference contained in the Centers for Medicare/Medicaid Services (CMS) Quality Insights Home Health Telehealth Reference 2006/2007 and is a contributing author of Implementing Outcome-Based Home Care; a Workbook of OBQI, Care Pathways and Disease Management.

Colleen is a  graduate of the University of Toledo and Covenant College with degrees in Nursing and Business. 
 

Melinda Huffman, BSN, MSN, CCNS, Principal of Miller & Huffman Outcome Architects, LLC is a nationally known author, writer and speaker. She’s a Certified Health Coach and a Co-Founder of the National Society of Health Coaches.  

 

Melinda has thirty years experience in hospital and home care administration, clinical supervision, quality management and staff development, with fifteen years devoted to healthcare outcomes management. She is a former Director of Disease Management for Amedisys and Director of Nursing Education, Motlow College in Lynchburg, TN. 

 

Melinda is the author of the homecare best seller: Implementing Outcome-Based Homecare: A Workbook of OBQI, Care Pathways and Disease Management and is receiving excellent reviews for her latest work, Home Health Outcomes, Pay for Performance, Disease Management: Gold Nuggets to Success!  She's a member of the national Technical Expert Panel and contributor for Quality Insights' Care Transitions Best Practice Interventions Packages. She writes regularly for nationally known peer-reviewed journals and newsletters on health coaching, evidence-based practice for chronic condition management and healthcare outcomes.

 

She received her BSN from the University of Alabama, Birmingham and a MSN in Cardiovascular Clinical Specialization from the Catholic University of America, Washington, DC.
